The HMO XS plan is mainly for New England residents. The HMO Network does not extend beyond New England.

The PPO plan is for residents outside of New England or for those of you in New England who prefer a PPO.

You will find descriptions of both plans below. You may choose either plan.
To get this plan rolling we need to fill out one of the following forms for everyone in the company. If you choose the HMO please put a Primary Care Physician (PCP) on the form. You have to have a PCP on the form or I cannot get you signed up. If you do not have a PCP you can still put one on the form and change it later when you do have one.

Since we are getting a new health plan, we also need new Schedule A's from everyone to comply with the section 125 cafeteria plan regulations set forth by the IRS. Please send in this form by April 1, 2008.
